Yes, you can view your certificates/policies by creating an online account. After creating your online account, any certificates/policies on which you are the owner and the insured will automatically be connected and viewable in your online account.
To create an online account, please visit our Member website and follow the on-screen instructions. You will then need to provide a unique user name, a valid email address, and a password, along with answering four security questions. After acknowledging the Terms and Conditions, click the Next button to continue.
On the next screen, you will need to provide the last four digits of your social security number, your date of birth and your mailing zip code, then click the Submit button. Once your information has been connected in our systems, you will be prompted to log in using the user name and password you created.
Upon logging in, you will see the certificates/policies on which you are the owner and the insured.
Currently, the only certificates/policies available are those where the member is the person insured and is also the owner. For example: Jane Doe has $50,000 of life insurance coverage on herself. She applied for the coverage and is the owner. Jane will see this certificate/policy information on the member website. Jane Doe also owns $25,000 of life insurance coverage on her son. Because Jane is not the insured, she will not see this certificate/policy on the member website.
Information on certificates/policies that are lapsed (inactive due to not paying premiums) over 45 days will not be available.
This means the site was unable to find your records based on the information you entered in your profile. To make sure that we have the correct information for you on file and to resolve this issue quickly, please contact Customer Service at 1-800-225-3108, or contact us online and include your name, certificate/policy number(s) and request.
You can make a one-time, non-recurring premium payment from your checking or savings account after creating an online account. Log in to your account, then click on the Payments tab at the top of the screen. Complete the online form and then click the Submit button.
You can have your payments automatically withdrawn from your bank account on a recurring basis for certificates/policies which you are both the owner and the insured. This is very convenient, eliminates late payments and may be eligible to receive a premium discount. Log in to your account and choose the Forms tab at the top of the page. Fill out the Pre-Authorized Payment Plan Form completely, print it, get the appropriate signature(s) for the account, and mail it to WoodmenLife.

To change your beneficiary for certificates/policies for which you are both the owner and the insured, log in to your account, then click the Forms tab at the top of the page. Fill out the Beneficiary Change Form completely, print it, get the appropriate signature(s), and mail it to WoodmenLife.

As the owner or payor of a certificate/policy, you can log in to your account, then click on the Forms tab at the top of the page to download the Pre-Authorized Payment Plan Form. Fill the form out completely with the new bank account information, print it, and mail it to WoodmenLife.

As the owner of the certificate/policy, you can log in to your account and complete the online Change of Address Form. Choose the Change Address/Phone link in the right column, enter the new information and select Save Changes to submit it. Traditional and Universal Life certificate statements are mailed each year by the 15th day of the anniversary month. Annual Reports are mailed each January for all annuity certificates. Mid-year reports are mailed in July for the following types of Annuity certificates with values of $5,000 or more: Flexible Premium Deferred Annuity, Single Premium Deferred Annuity and Non-Life Single Premium Immediate Annuity. These include Basic Annuities, IRAs, ROTHs, SIMPLEs, SEPs, and TSAs.

Tax Forms(1099 & 5498)

A reportable transaction was generated on your certificate.
Generally, for life insurance, distributions are taxable when the amount received is greater than the amount paid into the certificate.
Refunds can be used to purchase additional insurance, held on deposit or received in cash. When the amount of the funds received exceeds the amount paid into the certificate a 1099-R will generally be produced. The 1099-R will generate regardless of refund method.
Refunds held on deposit earn interest. The interest is reported to you each year similar to a savings account.
Interest earned on annuities is deferred until the funds are withdrawn. If a withdrawal is made and there is taxable gain on the certificate, a 1099-R will be issued.
Form 5498 is used to report contributions and rollovers to your retirement plan, including IRAs, Roth IRAs, SEPs and SIMPLEs. It is also used to report the fair market value of the plan at year end.
You are receiving this form because you made a contribution to your retirement plan in the current year for the prior year. Contributions can be made to your retirement plan up until April 15 of the current year for the previous year.
